Note

Only departments directly working with Digital Services are using form pages to embed form questions into SF.gov.

Forms allow users to enter information into a page.

Audience

How it works

The form questions and logic are created in Form.io. You can embed with help from Digital Services. Form.io forms can be embedded into SF.gov form pages.

Some forms have custom aliases. These currently don’t have human readable URLs by default.

Currently this supports three options for embedding forms: Acuity, FormBuilder, and Form.io. However we are moving toward Form.io for all forms.
