Use this content type for online forms

Goals

Forms allow users to enter information into a page.

Audience

How it works

The form questions and logic are created in Form.io. You can embed Form.io forms into SF.gov form pages.

Some forms have custom aliases. These currently don’t have human readable URLs by default.

Currently this supports three options for embedding forms: Acuity, FormBuilder, and Form.io. However we are moving toward Form.io for all forms.
